If you are looking for a subscribe form with an extra ordinary design then we have an excellent choice for you that is Easy subscription form. This smartly designed newsletter subscription form is fully responsive and comes with button hover effect built in HTML5 and CSS3.

Version 2.0 makes the subscription form work: the sliding label bars now stay aside once a field is filled, with a tick, name and email are validated, and a valid sign-up shows a thank-you without reloading the page. The look is refreshed with a glass card, taller fields and a full-width button (one CSS layer, easy to switch off). Adds an unsubscribe page (unsubscribe.html), a privacy page (privacy.html) and two ready-to-send HTML emails: you're subscribed (email-welcome.html) and you're unsubscribed (email-unsubscribed.html). The fonts are self-hosted.

What’s new

  1. v2.0Sep 2026
    • A working subscription form: name and email are required and checked, the message is optional, and a valid sign-up shows a thank-you that names the address, without reloading the page.
    • unsubscribe.html: leave the list with your email address, with a confirmation and a "subscribe again" link.
    • privacy.html: what the form collects, why, how long it is kept and how to leave.
    • Two ready-to-send HTML emails: email-welcome.html ("you're subscribed", with an unsubscribe link) and email-unsubscribed.html. Table-based and inline-styled for Gmail, Outlook and Apple Mail; no images to upload.
    • Connect the forms to your server: add data-endpoint="https://your-site.com/api/…" and they are POSTed there. Without it they run as a demo (each shows a "Demo only" line; delete those lines when you go live). Details at the top of js/main.js.
    • The title and form sit on a dark glass card, so they read clearly on the photo, with a larger title.
    • Taller fields with soft corners; what you type shows as written (no forced capitals) in the page's font.
    • The label bars are a lighter purple with an orange edge when active, and an orange tick at the end of every filled field.
    • A full-width Send button, and a comfortable card width on tablets.
    • All of it lives in one layer: remove the class "tv-modern" from <body> to get the original look back.
    • The label bars were meant to stay aside once a field had text, with a tick; that script needed a library that was never loaded, so it failed on load and a typed name disappeared under its bar again. It works now, without any library.
    • The tick used an icon font that was never included; it is an SVG now, and it no longer blocks the tap that opens the field.
    • The form had no field names and no handler, so it did nothing.
    • The form and the credit line were both positioned on the window, so on short screens they ran into each other; the page flows now.
