Basic Professional Sign Up Form – Do you need a professional sign up form for your site? Basic Professional Sign Up Form is a free widget template to give a simple yet professional look to your website. The free HTML template is developed using HTML5, CSS3 and bootstrap framework. The Sign Up form is responsive across all devices and compatible with all screen sizes. Download now!
Version 2.0 makes the sign-up card a working form: every field is labelled and validated, the password has a show/hide button and a live strength bar, and a terms checkbox is required before Get Started works. It adds the pages around it in the same design — a check-your-inbox page the form lands on (thank-you.html), a sign-in page (login.html), a forgot-password page (forgot-password.html) and a terms and privacy page (terms.html) — plus three ready-to-send HTML emails: confirm your email (email-confirm.html), welcome (email-welcome.html) and password reset (email-password-reset.html). The fonts are self-hosted and each form can be pointed at your own server with one attribute.
- A working sign-up form: every field has a label
- thank-you.html — where the form lands: "Check your inbox", naming the address it was sent to, with Resend Email. Marked noindex.
- login.html — sign in with email and password, show/hide password, "Keep me signed in" and a link to Forgot password? / Create an account.
- forgot-password.html — email field, validation and a "check your inbox" confirmation. Marked noindex.
- terms.html — the terms and privacy text the sign-up checkbox links to (placeholder details for you to replace).
- Three ready-to-send HTML emails, one per form: email-confirm.html (the confirm-your-email link)
- Connect it to your server: add data-endpoint="https://your-site.com/api/…" to a form and it is POSTed there; on success it goes to the form's data-redirect page
- The state list said "Dehli" and "Mumbai" (a city, not a state): now Delhi, Goa, Gujarat, Maharashtra and Other.

What’s new
- A working sign-up form: every field has a label, a name and an autocomplete hint; each empty or wrong field shows an error under it and focus jumps to the first one; the password has a show/hide button and a live strength bar (8+ characters); the phone number is checked; the terms box is required. The form is POSTed (v1 used GET).
- Three ready-to-send HTML emails, one per form: email-confirm.html (the confirm-your-email link), email-welcome.html and email-password-reset.html. Table-based and inline-styled for Gmail, Outlook and Apple Mail, no images, {{placeholders}} for your mailer.
- Connect it to your server: add data-endpoint="https://your-site.com/api/…" to a form and it is POSTed there; on success it goes to the form's data-redirect page. Without them the forms run as a demo (each shows a "Demo only" line — delete those lines when you go live). Details at the top of js/main.js.
- The state list said "Dehli" and "Mumbai" (a city, not a state): now Delhi, Goa, Gujarat, Maharashtra and Other.
- Only one of the six fields had a name and none had a label; two fields shared id="name" and the password and phone fields both used id="email". All fixed; the phone field is now type="tel".
- The page had two <h1>s and its h2 came before its h1. It is one <h1> ("Request Information", the first word small as before) and LOGO is a plain text block with the same look.
- Roboto now ships inside the page (woff2, the three weights used); nothing is fetched from Google. Unused CSS dropped.
- Meta description added to every page.
- Bootstrap 3.3.7 (a 35 KB customised build) -> Bootstrap 5 (CSS only; no JS bundle is needed) and one dependency-free js/main.js.
- Recolour in one place: --tv-accent / --tv-navy at the top of css/tv.css (the card's navy and the button's yellow also appear in css/style.css).
